Recommended referral process for all 3 services:
The Supported Life Style Hauraki Trust receives an enquiry from referring agency or family Referral information weed card is sent to The Trust The Trust meets prospective Life Styler and family/whanau on location for meeting and tour of facilities. At times it is useful for the Trust to carry out a visit to the applicants home to gather more information and assist with decision making and/or transition plans. Compatibility with other people within a Trust house is an important consideration. The applicant undertakes a trial in respite care (except day programme referrals) If residential care is being considered, then more than one respite care experience is recommended for getting to know each other. After the assessment period a decision is made by all parties in regard to an admission into The Trust.
